The Star Hospice of Vista Health Program provides medical care and emotional and spiritual support to terminally ill patients and their families. A team of highly trained nurses, social workers, pastoral ministers, nurse's aides and volunteers work under the direction of the patient's physician and the medical director. The team's ultimate goal is to work together to ensure the patient's varied and complex needs are met.

Am I A Likely Candidate for Hospice?

The following criteria will help to guide you to determine if Star Hospice of Vista Health is right for you and your family:

• You or a loved one has a terminal illness with a prognosis of six months or less.
• You or a loved one has a capable and willing primary caregiver in the home.
• You or a loved one has the desire (along with the family caregiver) to remain at home with established comfort care goals.

How Can We Seek A Referral?

To help everyone understand and face a terminal illness, it is best to seek hospice help at an early stage and anyone may refer a patient to Star Hospice of Vista Health. The physician must certify the terminal prognosis and agree to work with the hospice team in providing care for the patient. A physician's order is needed when the home care of an individual calls for the services of a skilled hospice team. The hospice team can address some or all of the needs an individual may require. Financing Hospice Care

Financing medical care for a loved one is a natural concern for anyone. The Medicare Hospice Benefit offers comprehensive hospice coverage. In addition, many private insurance plans offer similar hospice benefits. Please keep in mind, however, Star Hospice of Vista Health is available to anyone regardless of his or her ability to pay.
