If you are covered under Medicaid and are eligible to join an Accountable Care Entity (ACE), Vista Health System can help. Vista now is a member of Community Care Partners, an ACE designed to better serve the growing population of Medicaid patients.
Community Care Partners gives you access to all the services you need – medical, behavioral and even social support services. Through Community Care Partners, you can turn to Vista for the care you and your family need to stay healthy. Vista offers the services of physicians and healthcare professionals as part of this new network of providers that makes it easier for members of the community to receive high quality care.
Community Health Partners was formed by four healthcare providers to serve Medicaid patients in Lake County. The partnership includes Vista Health System, North Shore University Health System, Erie Family Health Center and the Lake County Health Department/Community Health Center.

Medicaid patients need to enroll in Community Care Partners in order to see a Vista primary care physician. Once enrolled, patients also can be seen at any of the four partner locations and can continue to see their same doctor if they are a part of the network. Patients are encouraged to have one primary care physician (PCP) and medical home to ensure the best continuity of care.
Medicaid patients should receive an enrollment packet from the Illinois Department of Healthcare and Family Services requiring them to enroll in a health plan. These plans are designed to provide for patients’ healthcare needs and help them coordinate their care.
It all starts at your doctor's office with Care Coordination. Your care team serves as your champion. They figure out what you need and then connect you to the right medical care and services.
If you are new to Community Care Partners, you have 90 days to change plans. After that period, you are required to remain in that plan for one year until your yearly enrollment period.
At the end of your enrollment year, you will get a letter from the Illinois Client Enrollment Broker (ICEB). The letter will say that you can change health plans if you want to. You will have 60 days to change. The letter will give you the dates that you can make the change. This 60-day period is called "open enrollment." You can change health plans during your 60-day open enrollment period every year.
